Forms will be filled out ONLY for established patients - at least 3 months in the practice

Forms that may be provided, at the clinician's discretion, could be 504 letters, ESA letters, etc

We DO NOT offer long-term disability certification, workers’ compensation, marijuana cards or forensic evaluations.

Short-term disability may be considered for established patients who are in a psychiatric day program that requires time off work. This will be at the clinician's discretion.

Any form requiring evaluation and more than 10 minutes of the clinicians time will be charged at a rate of $50.
